Carelinx caregivers

As a HealthSelect MA PPO member, you are eligible for 8 hours per month of in-home non-medical care needed with CareLinx. Learn more about Carelinx by calling (800) 337-3996 8:00 a.m.- 5:00 p.m. Monday – Friday  Local Time or online at www.carelinx.com/ers-ma.

Chronic Condition Care

Special programs are available to help you and your covered family members who are living with a chronic disease, like diabetes or heart disease. You can get personal attention and your doctors can get up-to-date information about your health.

FirstLine Essentials+

FirstLine Essentials+ is an Over-the-Counter (OTC) program that gives you a quarterly allowance to spend on over-the-counter care. With this program, you can shop for toothpaste, pain relief, vitamins, cough drops, and more. As a HealthSelect MA PPO member, you get a $40 quarterly allowance to purchase OTC health care products. HouseCalls

HouseCalls is a health and wellness program that comes to you. You are eligible for a yearly in-home visit from one of our health care practitioners at no extra cost. A HouseCalls visit is designed to support, but not take the place of your regular doctor’s care. Participation is voluntary and you can opt out at any time.

Nurseline*

NurseLine gives you 24/7 access to a registered nurse who can help you with sudden health concerns, answer questions about a medication, help you understand symptoms, an ongoing health condition or new diagnosis. Call NurseLine toll-free at (877) 365-7949; TTY 711, 24 hours a day, 7 days a week.

Personal Emergency Response System (PERS)

The Personal Emergency Response System (PERS) is an in‐home monitoring device that provides fast, simple access to help 24 hours a day, 365 days a year with the simple push of a button.

As a HealthSelect MA PPO member, you are eligible for a Phillips Lifeline medical alert device product of your choice. Alert systems come available in the form of a landline, wireless, or mobile GPS device. Post-discharge Meals

Our post-discharge meal delivery program provides freshly-made meals delivered to your home after you have been discharged from the hospital or skilled nursing facility. As a HealthSelect MA PPO member, you may receive up to 84 meals immediately following an inpatient hospital discharge or skilled nursing facility stay when referred by a UnitedHealthcare clinical advocate. To learn more call the UnitedHealthcare customer service advocates at (855) 853-0453 7 a.m – 7 p.m. CT Monday – Friday, 7 a.m. – 3 p.m. CT Saturday.

SilverSneakers

As a HealthSelect Medicare Advantage (PPO) member, you are eligible for a SilverSneakers® fitness membership at no additional cost to you. This fitness program includes access to 14,000+ participating locations1 and group exercise classes2 led by certified instructors. At-home kits are offered to members who want to start working out at home or for those who can't get to a fitness location due to injury, illness or being homebound. Transportation

Our Logisticare transportation program gives HealthSelect MA PPO members a ride to and from non-emergency, medical and pharmacy-related visits, such as doctors’ appointments, pharmacy trips and more. Virtual Doctor and Behavioral Health Visits

See a doctor or a behavioral health specialist using your computer, tablet or smartphone. With Virtual Visits, you’re able to live video chat from your computer, tablet or smartphone — anytime, day or night. First, you will need to register for an account with our Virtual Visit providers Doctor on Demand or Amwell, and then schedule an appointment. You also can download the Doctor on Demand or AmWell apps using your smartphone or tablet.
