Enroll in a plan

UC Blue & Gold and Health Savings  members

You should have received information from UC about enrolling into Medicare and how to access the required Medicare Assignment form. Complete and return the form with a copy of your Medicare ID card to the UC Retirement Administration Service Center (RASC). Taking action by the due date avoids delay in your enrollment into the UC Medicare Choice plan. If you have family members who are not Medicare-eligible, they will remain or move into  UC Blue & Gold (HMO), which is the “partner plan” for UC Medicare Choice. 

If you are eligible for premium-free Part A, UC requires you to enroll in the UC Medicare Choice plan. Please contact RASC at 1-800-888-8267, 8:30 a.m. – 4:30 p.m. PT, Monday – Friday with your questions.

If you do not want to enroll in the UC Medicare Choice plan, the annual UC Open Enrollment period is your opportunity to make a change to any of UC’s Medicare plans.


Members of other UC Medical Plans

During UC’s annual Open Enrollment period, you had the option to remain in your current plan or enroll in another UC Medicare plan – including UC Medicare Choice.

What am I enrolled in?

You're enrolled in the UC Medicare Choice plan. To learn more about your plan details, review plan benefits and costs.

What happens after UC Retirement Administration Service Center (RASC) sends the request for enrollment into the UHC Medicare Choice plan?

  1. We'll review your information and eligibility.

  2. After you're verified, your application will be sent to the Centers for Medicare & Medicaid Services (CMS) for approval. Approval may take up to one week.

  3. Once CMS approves your enrollment, you'll be sent a confirmation letter and your plan member ID card.

  4. Soon after, you'll receive your Plan Details booklet in the mail from UHC. It includes details about your UC Medicare Choice plan benefits.

  5. Once you receive your ID card, click on the “Register Now” link at to access view plan documents, find providers and pharmacies in your area, look up your Drug List (formulary) and review your Evidence of Coverage (EOC). 
    1.  Be sure to update your profile with your current address and phone number.
